Due to the 2020 outbreak of the Coronavirus (COVID-19);

Beauty by Rhina is taking extra precautions with every client to include

proper sanitation and disinfection procedures.

Symptoms of COVID-19 include fever, fatigue, dry cough, and difficulty breathing.

If you or any of your family members experience any of these symptoms, kindly reschedule your appointment.

Beauty by Rhina will be following procedures to prevent the spread of COVID-19 and cannot be held liable for

any possible exposure to the COVID-19 virus caused by misinformation on this form

or the health history provided by each client.

​

Prepayment for services is available and encouraged for contactless checkout. 

Additional time will be added between appointments to proper sanitize and to prevent other client contact. 

Guests accompanying those scheduled for services are discouraged unless for individuals with ambulatory special needs.

Each client is required to wash hands upon arrival and before leaving.

Beauty by Rhina will thoroughly clean hands and wear mask during makeup application for each client.  Disposable tools will be used where possible and product bottles used during service will be disinfected between clients. All tools and surfaces will be wiped thoroughly before and after each client.

 

By acknowledging below, client agrees to the above and releases Rhina Lopez of Beauty by Rhina  from all liability for any unintentional possible exposure or harm due to COVID-19.

Beauty by Rhina also acknowledges the same.
